I hereby authorize Doug Minor, United Pacific Mortgage, and/or Steve Ochs, owner/broker, Ochs Realtyto run a credit report for me.  It is understood that the purpose of this application is to receive a lender pre-qualification to purchase a home.  There is no obligation to use United Pacific Mortgage Company to represent me with my loan to purchase a home.  I will be provided with a copy of my credit report from all three major reporting bureaus.
